How to refer:
The patient will call LiveWell Rehab and Fitness, LLC at 484-433-2291 to obtain information and set up an appointment
Fax a PT prescription with the treating diagnosis to LiveWell Rehab and Fitness at 484-229-0277
A comprehensive PT evaluation will be performed and a written report will be sent to the provider. Medicare patients will need their plan of care signed and returned via fax to LiveWell Rehab and Fitness, LLC. 484-229-0277.
